Medical Consent Form Review

Information given is confidential and will only be used as per the terms of our Privacy Notice, which you can find on our Policies and Documents Page.
Do you give permission for an appropriately trained First Aider to give 'over the counter' antihistamines in the event of an emergency or incident where your child may be suffering from an allergic reaction?*
Do you give permission for an appropriately trained First Aider to administer analgesics (like Paracetamol) or other 'over the counter' medications to you child for minor ailments?*
Do you give permission for an appropriately trained First Aider to give First Aid to your child for minor ailments or injuries?*
Do you give permission for an appropriately trained First Aider to administer legally prescribed medication to your child as per General Practitioner's instructions?*
Do you give permission for an appropriately trained member of staff to accompany your child to a dental, general practitioner or hospital appointment, if necessary?*

This information will be used throughout your child's time at Slindon College, unless you inform us otherwise.
